5-8 Grade

2009-2010

Enclosed you will find the forms necessary for registering your child for sports this year. Please complete and turn them in to Mr. Burkee , your childs teacher, or the School Office by August 26. Direct any questions you may have to the Athletic Director, Mr. Burkee.

NOTE: During summer you may put these forms in Mr. Burkees church mailbox or the School Office.

SPORTS REGISTRATION FORM

Check each sport your child would like to participate in this school year. These results determine the number of teams we will have, the number of coaches needed, and tournament registrations.
Team sizes may be limited to make more playing time available for team members.
This is the only opportunity to register for 2009-2010 sports.

Pink Emergency Cards- Carefully review and complete the card. If you are using last years card, see below. New cards can be obtained in the office if needed.

Check old card and make necessary changes for:

1. Contact and insurance information

2. Years at the top of the card to read 2009-2010

3. Grade level to show 2009-2010 grade

4. Place your initials in upper right hand corner of card.

Forms

Carefully read the information on each form and sign each page. If you are using resigning last years forms, see below.

1. Carefully reread forms and make any needed changes.

2. Place your initials and the date at the bottom of each form to verify that you have read it and the information on it is correct.

*Physicals- just a reminder that all children entering 7^th grade need to have their physical forms turned in before practices begin
